Our interdisciplinary team—including clinicians experienced in dementia care, Care Navigators, and direct care staff—works together to deliver personalized, evidence-based services centered on your unique needs. 

Key Services We Provide Through the GUIDE Program in Miami and South Florida 

As part of the GUIDE Model, Home Healthy offers a full set of supportive services designed to make daily life easier, safer, and more manageable for you and your loved one: 

• Care Navigation — A dedicated Care Navigator helps you connect to medical care, community resources, meals, transportation, and emotional well-being support. 

• 24/7 Support Line — Round-the-clock access to our team for any urgent questions, worries, or crises that arise—day or night. 

• Caregiver Training and Education — Practical, evidence-based resources, ongoing guidance, and tailored training to help you feel more confident and less overwhelmed in your caregiving role. 

• Respite Services — Temporary relief for caregivers through in-home help, adult day programs, or short stays in a care facility (covered under the program for eligible participants). These breaks let you rest, run errands, or simply recharge. 

• Comprehensive Care Planning — Our team creates and regularly updates a personalized care plan that addresses the needs of both the person with dementia and the caregiver, in close collaboration with your primary doctor and specialists. 

• Connections to Community Resources — We screen for additional needs and make referrals to help with financial, logistical, or social challenges. 

These services work together to better manage dementia symptoms, improve coordination across healthcare providers, and help avoid unnecessary hospital or emergency visits—right here in the Miami and South Florida communities you call home.
