Understanding the Challenges of Dementia

Dementia affects millions of Americans—over 6 million today, with numbers expected to grow significantly in the coming decades. Living with dementia often means dealing with scattered care from different providers, which can lead to more hospital stays, emergency room visits, and difficulties managing other health conditions. 

For family caregivers—often spouses, adult children, or other loved ones—the responsibilities can feel overwhelming. You might handle personal care, medications, doctor appointments, and daily needs while juggling your own life. This can bring heavy emotional, physical, and mental strain. Without enough support, these challenges sometimes lead to earlier moves to nursing homes or other facilities, which isn’t always the best fit for everyone’s wishes or well-being.

What Is the GUIDE Model?

The GUIDE Model is a forward-thinking, voluntary nationwide program from CMS that started in July 2024. It focuses on helping people with dementia stay safely at home longer while giving much-needed support to their unpaid family caregivers. 

Through our participation in GUIDE, Home Healthy has built a dedicated Dementia Care Program. Our interdisciplinary team—including clinicians experienced in dementia care, Care Navigators, and direct care staff—works together to deliver personalized, evidence-based services centered on you and your family’s needs. 

Key Services We Provide Through the GUIDE Program

As part of GUIDE, we offer a full set of supportive services designed to make daily life easier and more manageable: 

  • Care Navigation — A dedicated Care Navigator helps you connect to medical care, community resources, and support for things like meals, transportation, or emotional well-being. 
  • 24/7 Support Line — Round-the-clock access to our team for any urgent questions, worries, or crises that come up. 
  • Caregiver Training and Education — Practical, evidence-based resources, ongoing guidance, and tailored training to help you feel more confident and less overwhelmed in your caregiving role. 
  • Respite Services — Temporary relief for caregivers, such as in-home help, adult day programs, or short stays in a care facility. (This provides essential breaks so you can rest, run errands, or recharge—covered under the program for eligible participants.) 
  • Comprehensive Care Planning — Our team creates and updates a personalized care plan that addresses the needs of both the person with dementia and the caregiver, working closely with your primary doctor and any specialists. 
  • Connections to Community Resources — We screen for additional needs and make referrals to help with financial, logistical, or social challenges. 

These services work together to better manage dementia symptoms, improve coordination across healthcare providers, and help avoid unnecessary hospital or emergency visits.
